Reminders for 2nd dose Appointments

  1. Fill out the immunization consent form for your 2nd dose appointment. Available via download on appointment link. 
  2. Bring with you
    1. Printed copy of your immunization consent form for your 2nd dose appointment
    2.  CDC COVID-19 Vaccination Card to your 2nd dose appointment
    3. Your medical and prescription card to clinic
