Step 1: Review Registration Forms Online

Register online from home or on computers located in the Campbell Union School District Administration Office, 155 N. Third Street, near downtown Campbell. Our Enrollment Staff is happy to help you. They are available by phone, email, and in person Monday through Friday (except holidays), with special hours from January 22 through February 15, 2019.

Step 2: Gather your Documents

Parents/guardians will need the following documents in order to complete the registration. View checklist

  1. Proof of Residency: We'll need proof that you reside in our district. Whether you own or rent your home, you are required to bring in 2 forms of address verification. All documents must be dated within the last 30 days. Examples of this are: property tax statement or payment receipts, rental agreement on company printed forms with phone numbers, utility service statement(s) and telephone bill (landline or cell). (Documents should have the student’s parent/guardian name). If you are living/sharing with another family you must provide (2) documents verifying residency along with our Declaration of District Residency form.
  2. Proof of Immunization: This is the immunization card from your physician or clinic, showing that the immunizations are up to date. (NOTE: If your child’s doctor has not given the immunizations yet because your child is too young, your packet can be completed by providing a letter from the doctor that has an appointment date and time, and that clearly states that the purpose of the appointment is for immunizations or TB test. This letter should have the doctor’s name and phone number on it. You will be asked to bring in the proof of immunization ASAP after this appointment.) Incoming 7th grade students must show proof of current Whooping Cough (Tdap) immunization.
  3. Proof of TB Skin Test: Proof of TB or TB Risk Assessment is due at the time of registration. All students must show Proof of TB Clearance before registering. For Kinder and TK students first entering school, proof of TB Clearance can be met by providing proof of TB Risk Assessment OR Tuberculin Skin Test (TST) or IGRA blood test completed within 12 months of registration. Transfer students from outside Santa Clara County (SCC) will be required, before registration, to provide proof of TB Risk Assessment OR TST OR IGRA blood test if they have resided outside SCC for greater than 12 months. (Proof of TB Skin Test (TST), TB Risk Assessment or IGRA blood test must be done by a health provider in the U.S.)
  4. Birth Verification: Birth Certificate, Passport or Hospital Birth Certificate.

Proof of Oral Health Exam: This must be turned in no later than May of the next school year but is not a requirement for registration

Step 3: Submit Completed Registration "Packet"

The registration "packet" consists of your completed form and the necessary documents above. We'll look it over to verify the completeness, make copies of original documents for our files, and give you a copy of the registration form for your files.
