Step 1: Review Registration Forms Online
Register online from home or on computers located in the Campbell Union School District Administration Office, 155 N. Third Street, near downtown Campbell. Our Enrollment Staff is happy to help you. They are available by phone, email, and in person Monday through Friday (except holidays), with special hours from January 22 through February 15, 2019.
Step 2: Gather your Documents
Parents/guardians will need to provide several documents in order to complete the registration.
- Proof of Residency in Our District: We need two recent documents (dated within the last 30 days) to confirm your address in our district. Examples include:
- Property tax statements or payment receipts
- Rental agreements with company contact details
- Utility bills or phone bills (cell or landline)
The documents must include the name of the student’s parent or guardian.
If you are living with another family, you’ll need to provide two documents verifying their address along with our Declaration of District Residency form.
- Proof of Immunization: You must provide a record from your doctor or clinic showing your child’s immunizations are up to date. (See list.)
If your child hasn’t received immunizations yet because they’re too young, you can turn in a letter from the doctor. This letter should include:- The appointment date and time
- A note saying the appointment is for immunizations or a TB test
- The doctor’s name and phone number
You’ll need to bring the immunization record to the school as soon as the immunizations are done.
For 7th-grade students: Proof of a current Whooping Cough (Tdap) immunization is required.
- TB Test or Risk Assessment: All students must provide proof of being cleared for tuberculosis (TB), due at the time of registration. This can include any of the following:
- A TB Risk Assessment completed by a doctor
- A TB Skin Test (TST)
- An IGRA blood test
The tests must be done by a healthcare provider in the U.S.
Kindergarten and TK students: TB clearance must be within 12 months of registration.
Transfer students from outside Santa Clara County: If you’ve lived outside the county for over 12 months, TB clearance must also be provided before registration. - Birth Verification: Birth Certificate, Passport or Hospital Birth Certificate.
Proof of Oral Health Exam: This must be turned in no later than May of the next school year but is not a requirement for registration
Step 3: Submit Completed Registration "Packet"
The registration "packet" consists of your completed registration form and the necessary documents above. We'll look it over to verify the completeness, make copies of original documents for our files, and give you a copy of the registration form for your files.
We will send you an official school placement notice in the springtime. Our top priority is to place your child in the school that serves the child's home. To request a different school, please refer to our school placement page for information about Open Enrollment.
